HippocrAItes launches DiabetesCare360: A development program for continuous, personalized care
DiabetesCare360 brings long-term monitoring and personalized treatment solutions into diabetes care. The development program is led by diabetologist Associate Professor Merja Laine, and its technological implementation is managed by HippocrAItes.
The DiabetesCare360 program supports continuous monitoring and personalized care through a collaborative model between physicians and nurses. The project is led by Associate Professor Merja Laine, MD, and focuses on the implementation of data-driven care models. The program aims to improve long-term treatment outcomes for people living with diabetes.
HippocrAItes launched the DiabetesCare360 program in summer 2025 to address the challenges of fragmented health data and discontinuous care.
“Too many patients fall through the cracks due to fragmented care pathways and lack of follow-up. Our goal is to create a reliable model that tracks treatment effectiveness over time and supports the patient throughout the entire care journey,” explains Laine. She is an Associate Professor at the University of Helsinki, CEO of MediConsulting MKL Ltd, and a nationally recognized expert in diabetes prevention and care development.
DiabetesCare360 is built on certified health technology
DiabetesCare360 leverages the Hippo Health Platform, a certified solution by HippocrAItes that consolidates an individual’s health data from various sources—such as national registries (e.g., Kanta) and personal monitoring devices—into a single secure view. The platform supports healthcare professionals in decision-making and enables continuity of care for the patient.
Amazon Web Services is HippocrAItes' technology partner. The Hippo Health Platform is built on AWS HealthLake cloud services, enabling scalable and secure health data management and analytics.
The Hippo Health Platform is officially certified as a Class A1 healthcare information system and has been approved for clinical use in Finland by Valvira. It meets the highest standards for data security, interoperability, and regulatory compliance. The platform is designed to be compatible with the European Health Data Space (EHDS) and supports cross-border continuity of care and innovation development across the EU.
The platform includes the Hippo Health Account, which gives individuals control over their health data, and Hippo Clinic, a professional interface for care teams to track goals, measurements and alerts in near real time.
Upcoming event: Future Health presents the first results of DiabetesCare360
HippocrAItes Oy will host the Future Health event on Thursday 2 October 2025, from 13:00 to 17:00, at Terkko Health Hub, Helsinki. The event marks the first public presentation of DiabetesCare360.
Moderated by renowned journalist Peter Nyman, the Future Health programme features a series of expert talks by healthcare professionals. Laine will also be present to share the first results of the DiabetesCare360 development program and explain how data-driven care facilitates more holistic treatment for people living with diabetes.
